Publication Agreements

The publication agreement form is at the bottom of this page. The corresponding author can sign for all authors, but must be aware that all authors must agree to this.

You may use an official digital signature.

The Publication Agreement should be signed, scanned, and uploaded as part of the Final Materials submission.

Please be sure to type or print all the information other than the signature.

Please do not make any changes to the document. Please do not make any marks of any kind on the form other than filling out the information requested.

Again, the corresponding author is responsible for making sure that all authors approve. Submitting the final publication agreement is legally binding for all authors.
